This mandatory consent form ensures we have a full understanding of your hair's history and your desired outcome, allowing us to create a personalized plan of action for your color service. Your service cannot be completed without this form.

Pre-service disclosure & client responsibility

At The Radiant Rose Salon, we pride ourselves on delivering high-end, customized color services while prioritizing the long-term health and integrity of your hair. To ensure the highest quality results, we ask that all clients review the following information regarding the chemical nature of our services.

1. Full disclosure of hair history

To avoid unexpected results, it is essential that we have a complete understanding of your hair’s history. You are required to disclose all professional and at-home color, chemical services, or treatments applied to your hair within the last three years. This includes, but is not limited to:

Box dyes or grocery store color products.
Henna or metallic-based dyes.
Previous professional corrective color services.
Any chemical straightening or smoothing treatments.

2. Chemical interactions & unpredictability

Hair with a history of color application (particularly box color) may react to professional lighteners and color differently. This can lead to unpredictable results or compromise hair integrity. We prioritize your hair's health and will adjust services if necessary.

At The Radiant Rose Salon, we are committed to the health and beauty of your hair. To ensure we provide the best service possible, we require all guests to acknowledge that hair chemistry is a complex, delicate process.

I confirm that I have provided an honest and complete history of all chemical services (including box dye, henna, or previous salon services) applied to my hair in the last three years. I understand that withholding this information may result in compromised hair integrity or unintended color results.

I understand that hair color and lightening are chemical processes, and that individual results may vary based on my hair’s unique texture, porosity, and previous product history.

I understand that if my hair exhibits signs of excessive damage or compromised integrity during a service, the stylist may need to stop or modify the service to ensure the long-term health of my hair.

I understand that I am financially responsible for the services rendered. If a service needs to be modified due to undisclosed hair history or hair integrity concerns, I will be responsible for the cost of the modified service.
