Before your appointment at The Radiant Rose Salon, we require specific intake, consultation, and liability forms to be completed. These forms are crucial for us to understand your hair history, preferences, and any sensitivities, ensuring a safe and personalized service. Please find the necessary forms below:
Hair services forms
For all hair-related services, including haircuts, styling, and color treatments, please complete the following forms. This helps us understand your hair's current condition and your desired outcomes.
- Haircuts and styling intake form: Details your hair length, texture, daily routine, and allows for photo uploads of desired styles.
- Hair color and color correction consent form: Requires a 2-year color history, elasticity checks, mandatory supplement/liver pill/detox screenings, and liability waivers.
- Chemical services consent form: For perms, smoothing treatments, chemical history, damage/supplement screening, and waivers.
Skin and beauty forms
If you are visiting us for waxing or facial services, please complete the respective forms to inform us of any relevant medical history or sensitivities.
- Waxing and facial consultation form: Informs us about retinoid use, skin sensitivities, sun exposure, and includes consent waivers.
Your hair goals and lifestyle
To help us achieve your dream look, please complete our general hair goals and lifestyle form. This helps us understand what you love, your daily struggles, desired changes, and how much time you dedicate to styling.
Please note our salon policy: No refunds will be issued for services. Any issues with your service must be reported within 48 hours of your appointment. Fixes are provided free of charge for up to one hour upon owner approval, with hourly charges applied thereafter.
